DISABILITY INCOME BENEFITS

SHORT-TERM DISABILITY - No cost to you

The short-term disability program offers financial protection when you are unable to work due to disability. You'll receive 66 2/3% of your weekly earnings, with a maximum of $1,000 per week. The benefit starts on the 1st day of disability from an accident or the 8th day from an illness and lasts for up to 26 weeks.

Important Note: Should you be out of work due to an approved Short-Term Disability leave, such leave is counted against the required 12-weeks of unpaid leave under Brubacher's FMLA policy.

VOLUNTARY LONG-TERM DISABILITY - Optional coverage you can buy

With Voluntary Long-Term Disability Insurance, you can choose to pay for additional coverage through weekly payroll deductions. If you become disabled, this benefit provides 60% of your base monthly pre-disability earnings, up to a maximum of $7,000 per month. The coverage starts after 180 continuous days of disability.

LIFE AND AD&D INSURANCE

LIFE INSURANCE - No cost to you

We provide $35,000 in Group Life and Accidental Death and Dismemberment (AD&D) Insurance. The life insurance benefits reduce to 65% at age 65 and 50% at age 70.

VOLUNTARY LIFE INSURANCE - You can choose to buy extra coverage

While we offer $35,000 of life insurance, you have the option to purchase more coverage through weekly payroll deductions. If you already have employee coverage, you can also get coverage for your spouse and dependents. For spousal coverage, you can buy increments of $5,000, up to a maximum of $250,000. If you sign up when first eligible, you can get a guaranteed issue of $10,000 without any medical questions. However, this amount cannot exceed 50% of the employee's coverage. Dependent coverage is available for dependents aged 14 days to 26 years, up to $10,000, but not more than 10% of the employee's coverage.

  • If you waive coverage when first eligible, future election will be subject to evidence of insurability.
